Safety analysis to address the functional safety requirements of ISO 26262 has been done by engineers for a long time and there are usually point tools in place to carry our HARA, FHA, FMEA, FMEDA and FTA. The most popular tool is MS Excel, but there are also specific tools like Fault Tree+, PTC Windchill, RAMS Com. and IQ FMEA.
Common errors using point tools are due to the lack of traceability, the inability to handle multiple design iterations, safety analysis not based on the actual design of the system and the analysis not being model based. Along with the errors, the use of point tools alone is incredibly inefficient, time consuming and expensive.
